Informed Consent Waiver

I hereby agree to be a participant in a program of strenous physical activity including various types of aerobic & anaerobic excercise and/or nutrtion recommendations as designed by Max Fitness, LLC. I acknowledge that I have been highly recommended to have a physical exam by a licensed physician prior to starting any exercise plan or nutrition recommendation. I hereby arm that I am in good physicial condition and do not suer any disability that would prevent or limit my participation in this exercise plan and/or nutrition recommendation, or cause or aggravate an existing medical condition/injury. I fully understand that illness, disease, injury and/or health related complications up to and including death, accidental or otherwise, may occur to myself as a result of participating in Max Fitness, LLC exercise plans or nutrition recommendations.

In consideration of my participation in any Max Fitness, LLC plans or recommendations for myself, my heirs and assigns, hereby release Max Fitness, LLC and its employees and owners from any claims, demands and causes of action arising from my participation in any exercise plan and/or nutrition recommendation.

I hereby release Max Fitness, LLC from any liability now or in the future including, but not limited to, illness, disease, injury or death, however caused, occuring during or after my participation.

I acknowledge that I have been highly recommended to have a bi-weekly tness assesment by a certied professional in order to monitor my progress.

This program is not intended to diagnose, treat, cure or prevent any existing or future disease and/or medical condition.
